Care Management:

I am a licensed Social Worker of more than 25 years here to help you and your family identify needs and find ways to meet the needs. I help find resources in order to make daily life easier.

A Care Manager is a helpful tool for families that live far apart:

Transitional Care:

Moving from one life stage to the next is often unclear and complex. We can help with the transition and assist in making it a smooth one with:

* service with medical decisions

* post hospital care, rehab, long term care, and assisted living

* assisting to ensure the move you make is the right one.

* checking in anywhere weekly to daily

Care Coordination:

Ensuring you receive the BEST care possible.

Organizing care, activities, information of a client and medical providers, family members, community care agencies, housing, and social activities.

Arrange/coordinate for day to day needs.

Forming the needed care team, educate, provide options, communication, evaluate, observe, develope care plans, monitor progress, and maintain quality of care.

As an elder advocate there are a vast variety of services offered from:

* coordination to find and obtain the services you need and deserve.

Senior Care:

Provide extra care in your private home or in assisted living. Extra assistance in the hospital or long term care center.

We come to you for your notary needs.
Your time, your place such as your home,
office, or a public location.

Signing made easy and convenient.

Transportation:

We will pick you up and take you to your appointment(s).
We will stay with you during your appointment.
We will go back with you to your appointment and take notes or assist with a video call with your family or friends if needed.
We have wheelchairs and walkers available to assist you during your transportation.
We will transport you back to your residence and make sure you get into your residence safely.
